The form consents and notice to be provided are attached to this order as Exhibit “1” hereto. IT IS FURTHER ORDERED that as to all additional Permitted Parties as set forth in this Order, pursuant to Paragraph 14 of the Bar Date Order, such Permitted Parties shall be required to execute the Confidentiality Agreement before receiving access to Sexual Abuse Proof of Claim Forms.
